To request an Appointment

  • Complete the online form for appointment Request.
  • Service available Monday to Friday from 8am until 14:00 or Practice reached the capacity.
  • If you can't use the internet or complete the form, call 0161 271 3040 or visit the GP surgery for help.
  •  All requests triaged by clinician with in two working days.
  • Patients will be offered an Appointment as per clinical needs.
  • This system ensures all patients with Urgent medical needs can get same-day appointments.

Home visits

  • Home visit service is for housebound and very ill patients.
  • Call 0161 271 3040 before 10:30am if you need a home visit.
  • Request a home visit only if you can't leave your home or are too ill to visit the practice.
  • The GP will decide if your condition needs a home visit and how urgent it is.
  • Many problems can be solved by talking to a doctor on the phone.You can ask for a "telephone consultation" instead of a home visit.
  • Community nurses can visit you at home if your GP refers you.
  • Health visitors will visit you at home if you recently had a baby or have a child under five and are new to the GP.

If you need help with your appointment

Please tell us:

  • If there’s a specific doctor, nurse or other health professional you would prefer to respond
  • If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • If you need an interpreter
  • If you have any other access or communication needs

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

You don’t always need to see a doctor. We offer a wide range of services provided by our Clinical Pharmacist, Advance Nurse practitioners, Social prescriber, First contact practitioners (MSK problem), Councillor, Focussed Care Worker, nurses and HCA’s. They often have more appointments available.

How long will my appointment be?

All appointments are 10 minutes long. 

Be on time!

Please arrive in good time for your appointment. The doctors may refuse to see a patient who is more than 10 minutes late.

Cancelling or changing an appointment

If you cannot keep an appointment, please let us know so we can offer your slot to someone else.

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • phone us on 0161 271 3040, Monday to Friday from 8am to 6.30pm
  • using the GP online system – via the link at the top of this page
  • reply CANCEL to your appointment reminder text message

We have strict rules for missed appointments. If you miss 2 appointments in 12 months without a good reason, you will get a warning letter explaining how missed appointments affect other patients. If you miss another appointment, you will be removed from our patient list.

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Extended Access

Appointments outside of core opening hours

Appointments are available Monday – Friday 6.30pm to 9pm and Saturday 9am to 5pm.

The service is provided at South Chadderton Health Centre, Eaves Lane, Chadderton OL9 8RG.
